General anaesthesia for neurosurgery.

چکیده

Anaesthesia for neurosurgery, especially intracranial operations, requires conditions which maintain the perfusion of the brain with blood. This perfusion results from the cerebral perfusion pressure: the difference between the arterial and the intracranial pressures. Anaesthesia must also facilitate surgical access by reducing brain volume and minimizing surgical haemorrhage. Other common problems arising in the conduct of neurosurgical anaesthesia include the diagnosis of neurological damage and the treatment of haemorrhage during operation. In the past decade, anaesthetic techniques have evolved which are designed to provide the necessary conditions for intracranial surgery, and incorporate the physiological information gained from the wealth of research on the effects of anaesthetic drugs and techniques on intracranial pressure, cerebral blood flow and cerebral metabolism. Many reviews and monographs reflecting the current interest in neuroanaesthesia have appeared in recent years (for example Hunter, 1964; Bozza Marrubini, 1965; Gilbert, Brindle and Salindo, 1966; Michenfelder, Gronert and Rehder, 1969; McComish and Bodley, 1971; Smith and Wollman, 1972; Gordon, 1975; Shapiro, 1975a, b). The particular hazards of intracranial hypertension, and of intracranial vascular surgery, have been stressed, as these form special risk groups.

BiSpectral Index (BIS) monitoring may detect critical hypotension before automated non-invasive blood pressure (NIBP) measurement during general anaesthesia; a case report.

A patient undergoing general anaesthesia for neurosurgery exhibited an unexpected sudden decrease in the BiSpectral Index (BIS) value to near-zero. This prompted the detection of profound hypotension using non-invasive blood pressure (NIBP) measurement and expedited urgent assessment and treatment, with the patient making a full recovery. Widely regarded as a 'depth of anaesthesia' monitor, thi...

Corticospinal direct response in humans: identification of the motor cortex during intracranial surgery under general anaesthesia.

The corticospinal direct (D) response to stimulation of the motor cortex exposed for intracranial surgery was recorded in 20 cases from wire electrodes inserted into the spinal epidural space. The D response was obtained from stimulation of restricted areas of the cerebral cortex, that is, the hand, trunk and thigh areas of the motor cortex. The D response was resistant to anaesthesia and unaff...

Induction of general anaesthesia with intraosseous injection of thiopental in rabbits

The technique of intraosseous (IO) infusion has been reemerged as an alternative route of intravascular access for the delivery of fluids and some medications. Complications of IO infusion occur rarely and include fractures and osteomyelitis after long-term use of IO access. The purpose of this study was to compare the efficiency of induction of anaesthesia by a standard intravenous route and a...

بررسی عوامل بیهوشی عمومی با ماسک در بیماران تحت عمل جراحی در یکی از بیمارستانهای وابسته به دانشگاه علوم پزشکی ایران(1372)

This research project is a field study to investigate the complications of general anaesthesia with mask in patients under surgery in a hospital affiliated to University of Medical Sciences Iran. The specific goals of this study are as follows:   -To determine some of the characters of patients under study.   -To determine the complications of general anesthesia with mask during surgery in on...

Effects of general anaesthesia on size of cerebrospinal fluid spaces during and after pneumoencephalography.

The mode of anaesthesia used during pneumoencephalography has a significant effect on the size of the cerebral ventricles 24 hours after the procedure. Post-encephalographic ventricular enlargement is less marked in patients examined under nitrous oxide anaesthesia. This appears to be related to passage of the gas into the ventricles during the encephalogram, and subsequent diffusion outwards. ...
